School-Based Health Care
School-Based Heath Centers (SBHC) provide comprehensive, acute care and preventive physical and behavioral health services for enrolled students on the school site. SBHC is a doctor's office inside of your child's school. These service help to shorten student;s time away from instructional time and decrease absences at school. The center is staffed with board-certified pediatricians and/or nurse practitioners, nurses, licensed behavioral health providers, and patient service representatives. School-Based Services Include​:

Frequently asked question (FAQ)
What services are provided at the school-based health center?
SBHC offers a variety of services including yearly well child exams, immunizations, sports physicals, acute care visits (i.e., sick visits, lacerations, injuries, etc.) and chronic care visits (i.e., asthma, ADHD, diabetes, etc.), behavioral health risk assessments, group and individual counseling.
How do students enroll in health center services?
Parents or legal guardians must sign a written consent form for their child(ren) to receive health center services. Consent forms only need to be completed once.
What if the student already has a primary care physician (PCP)?
The intention of the SBHC is not to take children away from their medical home but to supplement the care they are already receiving, and to reach out to children who may not have access to care. If necessary, the SBHC staff will communicate with the PCP to ensure the student receives the best care possible both in and out of school.
Is signing up for the SBHC mandatory?
Filling out the consent form and signing up your child(ren) is optional.
Will my child be seen at the school-based health center without my knowledge or without me?
Students with a signed parental consent form on file can receive services without a parent or guardian present. Parents may include, with the consent form, a written request to be notified by the health center staff before their child receives care. However, in the event of an emergency, necessary medical care will be provided to your child.
What is the cost?
There is not an out-of-pocket expense for SBHC services. The SBHC program will bill the student's health insurance for services provided on the site.
Parents / guardians will not incur a co-pay or other out-of-pocket expense. If the student does not have health insurance, there will be no charge to the student's family for services.
Does the SBHC administer immunizations?
The school-based health center can administer immunizations, upon request, to students who have a health center consent. In addition to the general health center consent, parents must also complete an immunization consent before a student is able to receive any vaccine. No student will receive a vaccine without the parent or legal guardian's prior knowledge and without an immunization consent that has been signed by the parent or legal guardian.
